has anyone come across this when u can not inpect that person who just killed you ??? Is this a hack or what ???
I mean u can inspect him but his model seem mess up and the cross hair only point at the center. :(

not really, I came across this a couple of times. It might be a small glitch in the hltv.dll and if anything, keep track of the names of the people you couldn't.
